ATHLETIC REGISTRATION INSTRUCTIONS 

Athletic Participation Online Registration

• Scroll down and select "Burrillville High School Athletic Registration 2026-2027"

• Log in or Create an Account and Validate Account through email if it is a new account

• Register each child participating.

  • • • Complete all required fields. Be sure to put the correct grade for 2026-2027.

      • Saved information from last school year will help expedite the registration process.

      • NEW: Upload a valid physical electronically. should be attached electronically. Hard copies will not be accepted. Students registering for Athletics must have a current physical examination submitted before the student is authorized to participate in practice/tryouts. Physicals are valid for 1 year from the date of the last exam.

• Assumption of Risk Form (Updated 2/4/24)

  • • • This must be completed once during the student's high school athletic career and brought to the main office of the school. It can be notarized in advance or brought to the main office not signed, waiting to be notarized by our staff.

• Proof of Insurance MUST be indicated on the registration form. Go to this link to register for School insurance.
